***Please note a new physical process for 2021-2022. 
This process will help streamline compliance to rules and expectations of the IHSAA and Northwest Allen County Schools.

In order to participate at Carroll High School students MUST have a completed and valid physical dated after April 1, 2021 to do summer strength & conditioning, open gyms, try-outs, competitions, practices, limited contact periods, etc. etc. etc.). Any physical dated before that will not be valid for the 2021-2022 season.

Please follow the steps found on the PDF attached or instructions below to fill out ALL requirements to have a completed physical.  In the end, you will have just the page that requires a Doctor’s signature to upload, scan in, or drop off to the CHS Athletic Department. (The single Doctor page is attached).  We prefer you uploading the document yourself.  This process will not allow you to miss information that is required and is often missed.

All forms are submitted through the Eventlink Community portal.


Completing Student Documents:

  • Create and/or login to the parent Eventlink Community account.
  • Select "Eventlink Registrations" from the top menu. You may be prompted to add a student. At this point you will add your student(s) in. Choose all sports that student plans to participate in.
  • Once completed, click on the Eventlink Registrations from the top menu.

A number of "Groups" may appear. Choose this one: 

  • Select "2021-2022 Physical Packet" icon.

Click on each icon: 



  • Fill in all materials asked for on each form.
  • Upload the doctor-completed physical where indicated. To do this, you will need to scan the physical completed by the doctor. Here are directions for scanning using phones. PDF
  • This does not need to be completed in one setting. You can return to Eventlink Registrations at a different time, select My Forms which will take you back to the form packet and continue filling out the information.


Currently athlete registrations only works on desktop/laptop computers. It is not currently supported for mobile devices!
